If you need additional help online, by phone, or via in-home care, Humana has options for you as well to help ease your caregiving responsibilities.

Caregiving support for Humana members

Humana members and their caregivers have access to services to help manage chronic conditions, meet health goals, navigate health plan information, and make the best use of prescription drug benefits. You may also be able to talk directly to a specially trained nurse* who can offer expert advice and guidance.
  • Call HumanaFirst® Health Planning & Support at 1-800-622-9529 to be referred to many support services. Some plans may even include our HumanaFirst® Nurse Advice Line* where you can get expert advice and guidance from a specially trained nurse — at no additional cost. You can also find out whether your loved one is eligible to be referred to programs that help manage chronic conditions.
  • Learn to live a healthier, more satisfying life with Humana Active Outlook®, a no-cost life enrichment program for Humana Medicare Advantage members. No matter what your personal health goals are, Humana can help.
  • The MyHumana web site lets members access their information anytime. Members can check on claims, get details about benefits, estimate medical costs, and more. To access this information on behalf of a loved one, see how you can be designated a caregiver.

Supplemental Programs When You Need More Help

SeniorBridge customized in-home support

Managing a loved one's ongoing care can be difficult. Sometimes you need help from experts. Humana is proud to introduce SeniorBridge, a national provider of in-home care management and caregiving self-pay services.

SeniorBridge care managers will listen and learn about your unique needs and preferences and present a personalized "care plan" including recommended type, frequency and cost of services to help your loved ones get the care they need. SeniorBridge will then coordinate the components of the care plan you choose to move forward.

You can also work with SeniorBridge to hire in-home caregiving services to assist with meal preparation, transportation, personal care, medication reminders and taking vital signs. As circumstances change, SeniorBridge will also be there to adapt the care plan as needed.
