You can pick anyone – your adult child, your partner, a sibling, even a close friend. Humana can share your Medicare information with your caregiver – but, by law, we need your permission first.
Just remember, your consent only allows Humana to share information with your caregiver. It doesn’t give your caregiver medical power of attorney – the right to make decisions about your care.
If you’ve chosen a caregiver, you can give Humana the go-ahead to share your medical information by mail or phone.
Call a Customer Care specialist at 1-800-4HUMANA (or 711 if you use a TTY), seven days a week, 8 a.m. - 8 p.m. If you decide to take care of this over the phone, you must be in the room when your caregiver talks to a Humana Customer Care.

After Humana’s gotten your form, you’ll receive permission to view your family member’s Medicare plan and all the support you need to become a knowledgeable caregiver – for free.
