Dental services
  • 1 comprehensive and dental cleaning every 6 months.

Acupuncture for chronic lower back pain
Up to 20 visits per year under the following circumstances:

For the purpose of this benefit, chronic low back pain is defined as:

  • Lasting 12 weeks or longer;
  • Nonspecific, in that it has no identifiable systemic cause (i.e. not associated with metastatic, inflammatory, infectious, etc. disease);
  • Not associated with surgery; and
  • Not associated with pregnancy

Your plan allows services to be received by a provider licensed to perform acupuncture or by providers meeting the Original Medicare provider requirements.

Over-the-counter (OTC) allowance
Up to $65 per quarter allowance enables members to purchase products that support common occurring conditions such as:

  • Pain relivers
  • Cough and cold relief medicine
  • First aid equipment that do not require prescriptions.

Unused amounts do not roll over to the next quarter.

Podiatry services
Up to 6 annual podiatry visits for the following:

  • Members in need of medical or surgical treatment of injuries and diseases of the foot
  • Members with conditions affecting the legs, such as diabetes

Post-discharge meals
14 refrigerated home-delivered meals following discharge from an inpatient or residential facility. Limited to 4 discharges per year.

Smartphone services

With a smartphone, you have easy access to health-related information and can stay connected to your care team and health plan. Any enrollee who qualifies for the Federal Lifeline program, will be eligible to receive a free cell phone with monthly talk minutes, text and data.

Smoking-cessation services

28 additional face-to-face visits with PCP or specialist offered. Refer to your Member Handbook for more information about your covered services.

Transportation services
This plan covers unlimited round trips per year to the pharmacy or medical supply providers right after doctor visits.
