To find providers accepting new patients, who speak your language and offer the services you need:

• Use our Find Care opens in new window service. 
• Sign in using your PathWays ID, date of birth, and zip code. 
• If you are not yet a member, or do not wish to sign in, you can navigate to “Search as a guest” and enter your address, city, county, or Zip code.  
  • Select the type of care you’re looking for. 
  • Choose a medical network. Select Indiana PathWays for Aging  
  • Enter your search criteria
