If you move

If you move after becoming a Humana Dual Integrated (HMO D-SNP) in Michigan member, you may no longer live in the same service area. If you move, please update your address within 30 days.

There are two ways to update your address:

  • Call the Social Security Administration toll-free at 800-772-1213 (TTY:1800-325-0778) Monday – Friday, 7 a.m. – 7 p.m., Eastern time, OR
  • Sign in to your Social Security account and update the address on file with Social Security.
