Humana Healthy Horizons in Oklahoma is proud to partner with the Oklahoma Health Care Authority (OHCA) to transition to a new healthcare model through the SoonerSelect program. The following SoonerCare populations may be eligible to pick a health plan starting February 1, 2024:
- Certain adults ages 19-64 (Medicaid expansion population)
- Children and newborns
- Parent and caretaker relatives
- Pregnant women
American Indian/Alaska Native members are considered voluntary and have the option to opt-in to a plan, like Humana Healthy Horizons in Oklahoma, or stay in the OHCA patient-centered medical home model.
