How to choose Humana Healthy Horizons

You have 90 days from your first day of SoonerSelect coverage to change your plan. If you want to switch from your current SoonerSelect plan and enroll in Humana Healthy Horizons in Oklanoma, you can:

  • Call OHCA at (800) 987-7767 (Option 5) to speak directly with a Choice Counselor.
  • Go to MySoonerCare.org
    1. Sign in to your account
    2. In the side menu on your benefits page, click the Change my plan/Choice provider link.
    3. On the plan selection page, click Change my plan/Provider
    4. Click see the full list of options.
    5. Select Humana Healthy Horizons in Oklahoma as the plan for each member in your family, and click Save.

Need to apply?

Before you can enroll in a SoonerSelect plan, you must apply for benefits. To apply:

  • Visit the Oklahoma Health Care Authority website and click Apply for SoonerCare online. There, you can find out if you’re eligible before applying. You can complete the application in the online portal.
  • Apply in person at a DHS office near you.
  • Call (800) 987-7767 (TTY:711) to speak with an OHCA Coice Coun­selor

 To apply, you will need information about yourself and the people in your home, including:

  • Full name
  • Social security number
  • Date of birth
  • Proof of citizenship
  • Income
  • Expenses (housing, childcare, etc.)
  • Assets (bank accounts, property, etc.)

SoonerSelect enrollment

Once the state determines that you qualify for SoonerSelect coverage, you must enroll in a plan. You can choose the plan you want, or the state will automatically enroll you into a plan.

When you first enroll in a SoonerSelect plan, you have 90 days to try the plan. During this 90-day period, you can switch for any reason to a new plan. After this 90-day period ends, you cannot change health plans until the next open enrollment period, unless you have an event that allows a change.

Open enrollment

The yearly Open Enrollment period gives SoonerSelect members a chance to change plans for any reason.

If you have Humana Healthy Horizons in Oklahoma and want to stay with us, you do not have to do anything.

If you want to switch to Humana Healthy Horizons in Oklahoma, you can use one of the options for switching health plans. If you change plans, you will be locked into that plan for 12 months.

Disenrollment for cause

If you are a SoonerSelect member and you want to change plans after you are locked in, you must have a state-approved for cause reason. For a list of state-approved for cause reasons, please refer to your Member Handbook  or call OHCA Choice Counseling at (800) 987-7767 (TTY:711).

Reinstatement

If you lose your benefits, you cannot be enrolled in Humana Healthy Horizons in Oklahoma. If you get your benefits back within 180 days (six months) of losing your benefit, and you were enrolled in Humana Healthy Horizons in Oklahoma when you lost your benefit, you will be re-enrolled in our plan. We will let you know in writing if you are re-enrolled in Humana Healthy Horizons in Oklahoma.
