The CAPABLE program can help Humana Medicare Advantage members in New York live safely in your home, longer.

What CAPABLE can do for you

If you have trouble with housework, managing your medicines or other daily tasks, you may be eligible for a program available through your Humana plan—at no additional cost to you.

The Community Aging in Place—Advancing Better Living for Elders (CAPABLE) program includes 6 home visits from an occupational therapist and 4 home visits from a registered nurse. Your care team will help you identify goals and take action, and a handy worker will help make your home safer with structural improvements. Overall, the program lasts about 4 months.

You may qualify for CAPABLE if you:

  • Are 65 or older
  • Have at least 1 qualifying chronic condition: COPD, coronary artery disease, heart failure, diabetes, hypertension, osteoarthritis or stroke
  • Received emergency care at the hospital in the past year
  • Show no signs of cognitive impairment
  • Struggle with 3 or more routine daily activities

CAPABLE helped Mrs. R, a 76-year-old retired music teacher, in the following ways:

  • Prepare meals easier by lowering the kitchen cabinets and installing mirrors above the stove
  • Feel less pain playing piano by building a higher piano bench and adding a pull chain to her LED light
  • Take safer baths by replacing the shower doors and frame and installing a tub transfer bench
