Humana Healthy Horizons in Florida Go365

For each eligible Go365 activity completed, you earn rewards that can be redeemed for e-gift cards in the Go365 in-app mall. Rewards earned through Go365^® have no cash value and must be earned and redeemed in the same plan year. Go365 for Humana Healthy Horizons is available to members who meet the requirements of the program.

To earn rewards:

• Download the Go365 for Humana Healthy Horizons app from the Apple App Store^® opens in new window  or Google Play^® opens in new window  on a mobile device
• Create an account to access and engage in the program
  • Members under the age of 18 must have a caregiver (i.e., parent or guardian) register on their behalf to participate. The caregiver must register for the program before adding a minor or dependent to access their Go365 information. They must also have valid proof of their relationship with the minor before they can be approved to access the program and assist with earning and redeeming rewards on behalf of the minor.
