If you are not happy with any part of your healthcare plan, Customer Care, your doctor, or a healthcare facility, you can send in a grievance. You also can appeal a claim or a denied service.

If you move

If you move after becoming a Humana Healthy Horizons in Florida member, you may no longer live in the Humana Healthy Horizons service area. If you move, please update your address within 30 days of moving.

To update your address:

  1. Call the Department of Children and Families (DCF) toll-free at 866-762-2237 (TTY: 1-800-955-8771), Monday – Friday, from 8 a.m. – 5:30 p.m., Eastern time, or
  2. Go to https://dcf-access.dcf.state.fl.us/access/index.do and update your address in your Automated Community Connection to Economic Self Sufficiency (ACCESS) account, or
  3. Call the Social Security Administration (SSA) toll-free at 800-772-1213 (TTY: 1-800-325-0778), Monday – Friday, from 7 a.m. – 7 p.m., Eastern time, or
  4. Go to https://secure.ssa.gov/RIL/SiView.do, and update your address in your Social Security account
