If you move
If you move after becoming a Humana Dual Fully Integrated (HMO D-SNP) in Illinois member, you may no longer live in the same service area. Please update your address within 30 days if you move.
There are two ways to update your address:
- Call the Social Security Administration toll-free at 800-772-1213 (TTY: 800-325-0778), Monday – Friday, 7 a.m. – 7 p.m., Eastern time; OR
- Sign in to your Social Security account
opens in new window and update the address on file with Social Security. - You must also update your address with Illinois Healthcare and Family Services (HFS)
