Baby and Me Meals

All

Up to 2 pre-cooked home-delivered meals per day for 10 weeks for pregnant members who are high-risk

Breast Pump

All

Pregnant and postpartum members can receive 1 non-hospital grade breast pump every 2 years

Convertible Car Seat and Portable Crib

All

Members can receive 1 free car seat or portable crib.  Members can also receive 1 free car seat or portable crib (that was not previously chosen) after completing postpartum assessment.

Smartphone Services

18 years of age and older

Members who qualify for the Federal Lifeline Program can get a free smartphone with monthly talk minutes, text, and data.

Healthy Food Produce Box

All

Members living with or at risk of living with a qualifying condition can receive up to 4 produce boxes per year. Qualifying conditions include the following:

  • Congestive Heart Failure (CHF)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Chronic kidney disease
  • End-stage renal disease (ESRD)
  • Cancer
  • Human Immunodeficiency Viruses (HIV)
  • Serious Mental Illness (SMI)
  • Diabetes Type I
  • Diabetes Type II

GED Testing

16 years of age and older

GED test preparation assistance, including a bilingual advisor, access to study materials, and test pass guarantee to provide members multiple attempts at passing the test.

Housing Assistance

18 years of age and older

Up to $500 per member per year (unused allowance does not roll over to the next year) to assist with the following housing expenses: 

  • Apartment rent or mortgage payment (late payment notice required) 
  • Utility payment for electric, water, and gas (late payment notice required) 
  • Trailer Park and lot rent if this is your permanent residence (late payment notice required) 
  • Moving expenses via licensed moving company when transitioning from a public housing authority

Newborn Circumcision

Over-the-Counter (OTC) Pharmacy Allowance

All

Up to $65 per quarter allowance enables members to purchase products that support chronic condition management such as:

  • Pain relievers
  • Diaper rash cream
  • Cough and cold relief medicine
  • First aid equipment that does not require prescriptions.

Unused amounts do not roll over to the next quarter.

Youth Development and Recreation

18 years of age and younger

Members can receive up to $250 annually for participation in activities such as:

  • YMCA
  • Boys and Girls Club
  • Swim lessons
  • Computer coding classes
  •  Music lessons

Post-discharge Meals

All

14  pre-cooked, home-delivered meals that can be stored in the refrigerator following discharge from an inpatient or residential facility up to 4 times per year

If you have been discharged from an inpatient or residential facility and have not been called to arrange delivery of meals, call Member Services at 866-432-0001 (TTY: 711), Monday – Friday, 8 a.m. – 8 p.m.

Sports Physical

6-18 years of age

1 sports physical per year

Complete a sports physical with doctor or other service provider

Tobacco & Vaping Cessation Coaching

12 years of age and older

Tobacco & Vaping Cessation Coaching is focused on helping members with stopping their usage of nicotine products.

The program also offers support for both over the counter (OTC) and prescription nicotine replacement therapy (NRT) for members aged 18 and older.

Weight Management Coaching

12 years of age and older

Weight Management Coaching helps members achieve or maintain a healthy weight.

Youth Academic Support

5-18 years of age

Members can receive up to 20 hours of tutoring over a 10-week period per year.

Vision Services

21 years of age and older

Members can receive 1 eye exam per year and up to $150 annual allowance to cover glasses, frames, and/or contact lenses.

Home-Based Asthma Interventions

All

Members living with asthma can receive up to $200 allowance per year for allergen free bedding, carpet cleaning and/or air purifier.

Housing Support for SMI Members

18 years of age and older

Members with Serious Mental Illness and eligible for Intensive Case Management who are homeless, at risk of becoming homeless, or transitioning from a behavioral health inpatient or residential facility without stable housing may be eligible for housing assistance up to $5,000. This once per lifetime assistance can cover:

  • Rent/mortgage
  • Security deposits
  • Utility deposits
  • Household furnishings/supplies
  • Moving expenses

Non-Medical Transportation

18 years of age and older

Up to 15 round trips (30 one-way trips) up to 30 miles for non-medical transportation per year to locations such as social support groups, wellness classes, WIC and SNAP appointments, and food banks.

Self-Monitoring Devices – Blood Pressure Monitoring Kit

21 years of age and older

Members can receive a Digital Blood Pressure Kit, which includes a cuff and monitor, every 3 years.

Produce Box for Maternal Care

All

Pregnant members that are between their 2nd trimester to 60 days after delivery can receive up to 4 boxes of in-season, nutritious food annually with approval.
