Baby and Me meals

All pregnant female members

Up to 2 pre-cooked home-delivered meals per day for 10 weeks for pregnant members identified as having a high-risk pregnancy

All female members

Female members can receive 1 non-hospital grade breast pump every 2 years, or 1 rental of a hospital breast pump if your baby has an inpatient stay in a neonatal intensive care unit (NICU)

Convertible car seat and portable crib

All pregnant female members

Pregnant members are contacted once we are notified that the mom is expecting. During the call or through a call to customer service, pregnant moms can confirm if a car seat or crib are needed prior to delivery.  Pregnant members who do not need the crib/car seat prior to delivery are asked to enroll and actively participate in our HumanaBeginnings Care Management program and complete a prenatal comprehensive assessment and then choose between a crib or a car seat. Upon completion of the postpartum assessment, and 1 follow up call the member can have the second item (portable crib/car seat). Members who are in need of the crib and car seat prior to delivery will receive the items and will also be contacted for a postpartum assessment along with a follow up call. This applies per infant, per birth.

Consent to participate and actively participate in HumanaBeginningsTM program, complete a comprehensive assessment, and complete 1 additional follow-up call within 56 days of enrollment with the program or identification of pregnancy

Smartphone services

All ages

1 Free smartphone through the Federal Lifeline Program, per household.  Members who are under 18 will need a parent or guardian to sign up.

This benefit covers per lifetime: 1 phone, 1 charger, 1 set of instructions, unlimited Talk, text and high-speed data, training for you and your caregiver at the first case manager orientation visit if you are enrolled in care management. Member must make 1 phone call or send 1 text message every month to keep benefit.

Member may qualify for enhanced benefits through the Affordable Connectivity Program (ACP) that provides unlimited minutes, 10 GB hotspot and unlimited data. You can opt into this benefit by contacting SafeLink at 1-800-SAFELINK or online at www.safelink.com/en/ACP11

Fresh produce box

All members

Up to 4 boxes of fresh fruits and vegetables per year. Member must be identified as food insecure, diabetic, suffering from heart failure, or suffering from hypertension.

GED testing

16 years of age and older

GED test preparation assistance, including a bilingual advisor, access to guidance and study materials, and unlimited use of practice tests. Test preparation assistance is provided virtually to allow maximum flexibility for members. Also includes test pass guarantee to provide members multiple attempts at passing the test.

You can be ages 16 - 18 if you have a South Carolina Verification of School Withdrawal form completed by the principal or attendance supervisor of the last school attended.  The GED may be taken at age 19 + without the South Carolina Verification of School Withdrawal form.

Haircuts for kids

Grades K-12

Haircuts for Kids Grades K-12 1 standard haircut for members in grades K-12 valued at $20, who upload a photo of their school registration form or school ID or class schedule, redemption period July 2024 through September 2024.

Housing assistance

18 years of age and older

Members who qualify may receive up to $750 per lifetime to help with the following housing expenses:

  • Apartment rent or mortgage payment (late payment notice required)
  • Utility payment for electric, water, gas, or internet (late payment notice required)
  • Trailer park and lot rent, if this is your permanent residence (late payment notice required)
  • Moving expenses via licensed moving company when transitioning from a public housing authority

Plan approval required.

- Member must complete Health Risk Assessment (HRA)

- Member must not live in a residential facility or nursing facility

- Funds will not be paid directly to the member

- If the bill is in the spouse's name, a marriage certificate may be submitted as proof

Newborn circumcision

29 days old through 12 months

Covered in the hospital starting when the baby turns 29 days old through 12 months of life

Over-the-counter (OTC) pharmacy allowance

All Ages

Up to $30 per quarter allowance enables conditions such as:

  • Pain relivers
  • Diaper rash cream
  • Cough and cold relief medicine
  • First aid equipment that do not require prescriptions.

Unused amounts do not roll over to the next quarter. members to purchase products that support common occurring  

Youth development and recreation

18 years of age and under

Member can receive reimbursement of up to $250 annually for participation in activities such as:

- YMCA

- Boys and Girls Club programming

- Swim lessons

- Computer coding classes

- Music lessons

Post-discharge meals

All members

Up to 14 home- delivered meals  following discharge from an inpatient or residential facility. Limited to 4 discharges per year.

Sports physical

Ages 6-18

Tobacco & Vaping Cessation Coaching

12 years of age and older

Tobacco Cessation Program is focused on tobacco and vaping cessation coaching for member aged 12 and older. The program is designed as a 6-month engagement for a total of 8 coaching calls, but member have 12 months to complete the program if needed.   

Humana’s tobacco and vaping cessation health coaching program offers support for both over the counter (OTC) and prescription nicotine replacement therapy (NRT).

 

Weight Management Coaching

12 years of age and older

Weight management Coaching Program delivers weight management intervention for members who are 12 and older. Upon receiving physician clearance, members can complete six (6) weight management coaching sessions with Health Coach; approximately one call per month for a period of six (6) months.

Youth academic support

Members in kindergarten through 12th grade

Online tutoring services for 2 hours per week

Vision services

21 years of age and older

  • Comprehensive vision exams every year (12 months)
  • 1 set of eyeglasses (lenses and frames) or contacts every
  • 2 years
