If you move after becoming a Humana Healthy Horizons in South Carolina member, please call Healthy Connections right away at 888-549-0820. To qualify for Medicaid benefits in South Carolina, you must be:

  • A South Carolina resident, and
  • A U.S. citizen or legally residing non-citizen
