South Carolina Medicaid: Coverage renewal

Everyone has unique needs and deserves unique care. Humana Healthy Horizons® in South Carolina is a Medicaid health plan available in South Carolina. After qualifying for Medicaid, you must renew your coverage each year. Renewal means you get to keep your coverage and benefits. If you don’t renew your coverage, you can lose your healthcare coverage and benefits.

Renewing your coverage

The South Carolina Department of Health and Human Services (SCDHHS) will send you an Annual Review Form in the mail. Complete and return your form by the due date listed. If the form is not returned in time, you will receive a notice letting you know when your Medicaid coverage will end.

You can renew your Medicaid coverage:

  • By phone: Call the Healthy Connections Medicaid main customer service line at 888-549-0820.

  • By fax: Applications and supporting documents may be sent by fax to 888-820-1204, or electronically to 8888201204@fax.scdhhs.gov .
  • By mail: Send your renewal application to
