Vision benefits for Humana Healthy Horizons® in Florida members include:  

  • A yearly eye exam
  • Eyeglasses
  • Contact lenses
  • Prosthetic eyes
  • Eye disease testing and care

For members older than 21, benefits also cover:

  • One set of glasses frames per year*, OR
  • One six-month supply of contacts

* You will pay any cost that exceeds $75 for luxury frames)

You may need to get approval before getting some services. This approval is called prior authorization or preauthorization.
