Humana Healthy Horizons® in Florida helps our pregnant and new moms get the care they need. Through our HumanaBeginnings® care management program, pregnant members get access to care managers, rewards for healthy actions, extra support, and more.

All moms need help before and after giving birth. Our HumanaBeginnings program:
- Is for our pregnant members
- Includes rewards for going to the doctor and getting certain screenings and vaccines
- Provides the help we know you need before and after you give birth
Member benefits:
Welcome to HumanaBeginnings! Members in our program get:
While you’re pregnant:
- Support from nurses trained to help during pregnancy
- Referrals for other treatment and community services
- Education and support for you and your partner or support systems
- Transportation to and from doctor appointments
- Extra help or support with understanding your Humana Healthy Horizons in Florida Benefits
After your baby is born:
- Coordination with your hospital, lactation, and newborn team
- Benefit information
- Newborn support care
- Referrals for other treatment and community services
- Family-planning information and services
- Education and support from partners
- Transportation to and from doctor appointments
Members can also:
- Earn rewards
- Get a free hospital grade or non-hospital grade breast pump, if their OB-GYN submits a referral to a contracted vendor
- Get their son circumcised, if so desired, at no cost
- Access nutrition counseling sessions
- Get meals delivered to their home after their baby is born
- Get money back every month to use for over-the-counter medicine

Go365 for Humana Healthy Horizons® is a wellness program that offers you the chance to earn rewards for taking healthy actions, like:

Health Risk Assessment
We ask all members to complete an Health Risk Assessment (HRA). The HRA is your chance to:
- Tell us about your health and well-being
- Tell us about your health goals
- Share information about your health related needs
- Get paired with a care manager who can help you reach your health and well-being goals
Completing your HRA is important for our pregnant members, especially during the first 60 days of enrollment in Humana Healthy Horizons in Florida. To complete your HRA:
- Fill out the HRA and send it back to us in the envelope you got with your New Member Welcome Kit, or
- Submit your HRA through Go365 for Humana Healthy Horizons and earn:
- $20 in rewards, if you complete the HRA within 90 days of enrollment, or
- $10 in rewards, if you complete the HRA after 90 days of enrollment

- Register for a MyHumana

This reward is available once per new enrollment in Humana Healthy Horizons in Florida.

Urgent maternal warning signs
During and after pregnancy, you and your body will go through many changes. Most of those changes will be normal. Some changes may not be normal. They could be a sign of serious problems that can cause long-term effects or even death.
You may not know how to tell the difference between normal and abnormal changes. But, you know when something feels wrong. If you are worried about symptoms or changes, talk to your doctor. We want you to make sure you get the care you need.
