Take advantage of the extras you get as a Humana Healthy Horizons® in Kentucky enrollee. Below, you’ll find information about each available benefit.

Cellphone Services
Free cellphone through the Federal Lifeline Program, per household. Enrollees who are under 18 will need a parent or guardian to sign up.
This benefit covers:
Calls to Humana Enrollee Services for health plan assistance and 911 for emergencies are unlimited, even if you run out of minutes.
You must make at least 1 phone call or send 1 text message every month to keep your benefit.
You also may qualify for enhanced benefits through the Affordable Connectivity Program (ACP). These enhanced benefits include:

Criminal Expungement Services
Reimbursement of up to $340 for criminal record expungement, as allowed per KYCourts.gov, per lifetime

An all-inclusive online program with tools to assist enrollees with passing the GED test.

Go365 Wellness Coaching, Tobacco Cessation
For all enrollees ages 12 and older, up to 8 health coaching/cessation support calls within 12 months from enrollment date.
For enrollees ages 18 and older, nicotine replacement therapy is available upon request.
This program will have two opportunities where enrollees can earn rewards. Enrollee must opt into Go365 mobile app in order to be eligible to receive reward.
Go365 Wellness Coaching, Weight Management
Enrollment in Weight Management Program, completion of a well-being check-up and form with their primary care provider (PCP), completion of 6 total wellness coaching calls within 12 months of enrollment date or return of the PCP form.
This program will have two opportunities where enrollees can earn rewards. Enrollee must opt into Go365 mobile app in order to be eligible to receive reward.
Home-Delivered Fresh Fruits and Vegetables
Up to 4 boxes of fresh fruit and vegetables per year for enrollees under care management for diabetes or heart conditions.
Care Manager approval required.
Meals – Post-Discharge
Up to 10 home-delivered meals following discharge from an inpatient or residential facility, limit 40 meals per year (up to 4 discharges).

Pregnant female enrollees who complete 6 prenatal visits and participate in the HumanaBeginnings™ care management program are eligible to choose one of the following per infant per pregnancy:
Remote Monitoring Devices
Enrollees that are under care management for congestive heart failure (CHF) or end stage renal disease (ESRD) can receive 1 weight scale every 3 years and enrollees that are under care management for hypertension or pre-eclampsia can receive 1 digital blood pressure cuff every 3 years.

Vida Health Smartphone app for Diabetes Management
For enrollees with Type 2 diabetes who are not already receiving care management services, unlimited access to an innovative digital therapeutic smartphone application for diabetes management through Vida Health.

Female pregnant enrollees and enrollees with a child up to 1 year of age, can get:

1 sports physical per year for enrollees between the ages of 6 and 18
Workforce Development Program
, PDF opens new window will provide up to 12 months of assistance to support each participant in planning for the future (e.g., education, training, financial counseling) and engaging in and maintaining meaningful work (e.g., job support and retention coaching), and 3 round trip bus vouchers for transportation when enrolled in the program.

A Workforce Development Program Employment Coach contacts prospective enrollees to:
After enrollment into the program, enrollees are assigned an Employment Coach.
Enrollee reimbursement for childcare of $40 max per quarter, up to 4 times per year, for caretakers seeking job opportunities; enrollee must participate in the Humana Workforce program to be eligible for reimbursement consideration.

According to Feeding America, more than 37 million people in the United States (or 1 in 9 Americans) are unable to access healthy food. Accessing healthy food, especially during the ongoing COVID-19 pandemic, has been difficult for many people. There are programs that can help.
