To support your journey to good health, we make available extra programs and services that can help make your life easier.

Extras for our enrollees

Take advantage of the extras you get as a Humana Healthy Horizons™ in Kentucky enrollee, including:

Benefit Description

Cell Phone Services, PDF opens new window

Free Tracfone through the federal program, per household. Enrollees who are under 18 will need a parent or guardian to sign up

This benefit covers:

  • 1 Phone
  • 1 Charger
  • 1 Set of instructions
  • 350 minutes of talk time per month
  • 3 GB of data per month
  • Unlimited text messages each month
  • Training for you and your caregiver (at the first case manager orientation visit)
  • Free calls to Humana Enrollee Services for health plan assistance, and to 911 for emergencies (even if you run out of free talk time)

You must make at least 1 phone call or send 1 text message every month, to keep your benefit. Enrollees under case and disease management may be eligible for unlimited minutes and 4 GB of data upon Plan approval

Child Care Assistance

Enrollee caretakers age 21 and older participating in the Humana Workforce program may be eligible for reimbursement of up to $40 per quarter for child care during job-seeking opportunities, limited to 4 times per year

Criminal Expungement Services

For enrollees age 21 and older, reimbursement of up to $300 for criminal record expungement, as allowed per KYCourts.gov, per lifetime. You are responsible for the cost of the expungement certificate.

Dental Services - Adult

1 additional cleaning per year for enrollees age 21 and older

Doula Services

Per pregnancy, for enrollees: Doula assistance to provide emotional and physical support to the laboring mother and her family, 2 prenatal visits, 2 postpartum visits, and 1 visit for delivery assistance

GEDWorks

For enrollees age 18 and older: GED test-preparation assistance, including a bilingual advisor, access to guidance and study materials, and unlimited use of practice tests. Test preparation assistance is provided virtually to allow maximum flexibility for enrollees. Also includes test pass guarantee to provide enrollees multiple attempts at passing the test.

Moms First - Portable Crib

1 portable crib, per pregnancy, for female enrollees who complete at least 7 prenatal visits (Moms First enrollment required to qualify for benefit)

Non-pharmacological Pain Management Alternatives

For enrollees ages 21 and older who have chronic pain:

  • Up to 24 acupuncture visits, and
  • Up to 26 additional chiropractic visits per year above the base benefit of 26 visits

Post-Discharge Meals

Up to 10 home-delivered meals for enrollees, after discharge from an inpatient or residential facility. Limited to 4 discharges per year, for a total of no more than 40 home-delivered meals (10 per discharge)

Smartphone App for Diabetes Management

For enrollees 18 years and older with diabetes, unlimited access to an innovative digital therapeutic smartphone application for diabetes management

Smartphone app for Prenatal Care, Breastfeeding, and Newborn/Infant Care Assistance

For pregnant enrollees and enrollees with a child up to 1 year of age, unlimited access to Pacify, a smartphone application that provides 24 hour a day, 7 days a week access to a proprietary, video-enabled call-routing system that allows enrollees to connect with a lactation consultant or a physician extender for on-demand assistance

Sports Physical

1 sports physical per year for enrollees ages 6 to 18

Tobacco Cessation

Access to the Wellness Coaching Team using Go365 for Humana Healthy Horizons in Kentucky for participation in the tobacco- and vaping-cessation program.

  • For all enrollees ages 12 and older, up to 8 health coaching/cessation support calls within 12 months of the first coaching session
  • For enrollees ages 18 and older, nicotine replacement therapy upon request

Vision Services – Adult

Eyeglasses (frames and lenses) every 24 months for enrollees ages 21 and over
