To support your journey to good health, we make available extra programs and services that can help make your life easier.

Extras for our enrollees

Take advantage of the extras you get as a Humana Healthy Horizons® in Kentucky enrollee. Below, you’ll find information about each available benefit.

Benefit Description

Cellphone Services

Free cellphone through the Federal Lifeline Program, per household. Enrollees who are under 18 will need a parent or guardian to sign up.

This benefit covers:

  • 1 phone
  • 1 charger
  • 1 set of instructions
  • 350 minutes per month
  • 4.5 GB of data per month
  • Unlimited text messages per month
  • Training for you and your caregiver at the first care manager orientation visit if you are enrolled in care management

Calls to Humana Enrollee Services for health plan assistance and 911 for emergencies are unlimited, even if you run out of minutes.

You must make at least 1 phone call or send 1 text message every month to keep your benefit.

You also may qualify for enhanced benefits through the Affordable Connectivity Program (ACP). These enhanced benefits include:

  • Unlimited minutes
  • 5 GB hotspot
  • Unlimited data

You can opt into this benefit by contacting SafeLink at 877-631-2550 or visiting Safelink online.

Benefits are subject to change by the FCC under the Lifeline program.

Criminal Expungement Services

Reimbursement of up to $340 for criminal record expungement, as allowed per KYCourts.gov, per lifetime

To qualify for Criminal Expungement Services:

  • Open to Humana Healthy Horizons in Kentucky enrollees older than 18
  • Enrollee must have a Class D felony
  • Enrollee must meet the Commonwealth of Kentucky’s guidelines for expungement

Once expungement certificate is received:

  • Enrollee has 30 days to file a petition for expungement
  • Enrollee pays the $50 non-refundable filing fee

When the enrollee’s expungement order is granted:

  • Enrollee pays additional fee of $250.

To request help with the expungement application process, enrollees must contact one of the following organizations or resources:

  • Region 1: Kentucky Legal Aid
  • Region 2: Kentucky Legal Aid
  • Region 3: Louisville Urban League, Reily Reentry Project
  • Region 4: Kentucky Legal Aid
  • Region 5: Legal Aid of the Bluegrass
  • Region 6: Legal Aid of the Bluegrass
  • Region 7: Legal Aid of the Bluegrass
  • Region 8: ApalReD
  • Statewide:

Doula Services

Doula assistance, per pregnancy, to provide:

  • Emotional and physical support to the laboring mother and her family
  • 4 prenatal visits
  • 4 postpartum visits
  • 1 visit for delivery assistance

GED Testing

An all-inclusive online program with tools to assist enrollees with passing the GED test.

Assistance includes:

  • One-on-one bilingual advisor
  • Access to guidance and study materials
  • Unlimited use of practice tests
  • Virtual test preparation assistance (to provide the greatest flexibility for participation)
  • Test pass guarantee (enrollees can take the GED multiple times until they pass it)

To be eligible for this benefit you must:

  • Be at least 18 years old
  • Not have a high school diploma

Note:

Go365 Wellness Coaching, Tobacco Cessation

For all enrollees ages 12 and older, up to 8 health coaching/cessation support calls within 12 months from enrollment date.

For enrollees ages 18 and older, nicotine replacement therapy is available upon request.

This program will have two opportunities where enrollees can earn rewards. Enrollee must opt into Go365 mobile app in order to be eligible to receive reward.

  • $25 in rewards for enrollees who complete 2 calls within the first 45 days of enrollment in the coaching program, once per year
  • $25 in rewards for enrollees who complete 6 additional Wellness Coaching calls (total 8) within 12 months of the first coaching session, once per year

Go365 Wellness Coaching, Weight Management

Enrollment in Weight Management Program, completion of a well-being check-up and form with their primary care provider (PCP), completion of 6 total wellness coaching calls within 12 months of enrollment date or return of the PCP form.

This program will have two opportunities where enrollees can earn rewards. Enrollee must opt into Go365 mobile app in order to be eligible to receive reward.

  • $10 in rewards: Enrollment in the weight Management Program
    • Completion of Wellbeing checkup with primary care provider
    • Submission of PCP form
  • $30 in rewards: Completion of the program
    • 6 wellness coaching calls within 12 months of the first coaching session

Home-Delivered Fresh Fruits and Vegetables

Up to 4 boxes of fresh fruit and vegetables per year for enrollees under care management for diabetes or heart conditions.

Care Manager approval required.

Meals – Post-Discharge

Up to 10 home-delivered meals following discharge from an inpatient or residential facility, limit 40 meals per year (up to 4 discharges).

Mom’s Meals will:

  • Call enrollees identified as discharged from an inpatient or residential facility
  • Offer the post-discharge meals benefit

Portable Crib or Car Seat

Pregnant female enrollees who complete 6 prenatal visits and participate in the HumanaBeginnings™ care management program are eligible to choose one of the following per infant per pregnancy:

  • A portable crib, or
  • A car seat

Remote Monitoring Devices

Enrollees that are under care management for congestive heart failure (CHF) or end stage renal disease (ESRD) can receive 1 weight scale every 3 years and enrollees that are under care management for hypertension or pre-eclampsia can receive 1 digital blood pressure cuff every 3 years.

Care Manager approval required.

Vida Health Smartphone app for Diabetes Management

For enrollees with Type 2 diabetes who are not already receiving care management services, unlimited access to an innovative digital therapeutic smartphone application for diabetes management through Vida Health.

After downloading and registering for the app, enrollee:

  • Takes a health and wellness assessment
  • Will pick a health coach with whom to work
  • Will start a one on one or group coaching sessions

With this app, enrollees can:

  • Access videos and helpful articles
  • Enter biometric (health) information in a secure data environment

The program includes:

  • 12 weeks of structured outreach, during which enrollees participate in a weekly program and interact with their respective health coaches.
  • 12 weeks of maintenance, during which enrollees practice self-care that their respective health coaches monitor

After the full 24-week program, enrollees graduate.

Pacify Smartphone App for Prenatal Care, Breastfeeding, and Newborn/ Infant Care Assistance

Female pregnant enrollees and enrollees with a child up to 1 year of age, can get:

  • Unlimited access to Pacify, a smartphone application that provides 24 hours a day, 7 days a week access to a proprietary, video-enabled call-routing system.

Support from a lactation consultant or a physician extender for on-demand assistance.

Sports Physicals

1 sports physical per year for enrollees between the ages of 6 and 18

Workforce Development Program

The Humana Workforce Development Program, PDF opens new window will provide up to 12 months of assistance to support each participant in planning for the future (e.g., education, training, financial counseling) and engaging in and maintaining meaningful work (e.g., job support and retention coaching), and 3 round trip bus vouchers for transportation when enrolled in the program.

Limited space available.

Humana identifies enrollees who may meet the criteria to participate in this program based on:

  • Their responses to questions in the Health Risk Assessment (HRA)
  • Referral from a Humana care manager
  • Enrollee self-referral

A Workforce Development Program Employment Coach contacts prospective enrollees to:

  • Complete an employment assessment
  • Determine if enrollee meets participation criteria
  • Determine interest

After enrollment into the program, enrollees are assigned an Employment Coach.

Enrollee reimbursement for childcare of $40 max per quarter, up to 4 times per year, for caretakers seeking job opportunities; enrollee must participate in the Humana Workforce program to be eligible for reimbursement consideration.

Go365 Humana Healthy Horizons

Go365 for Humana Healthy Horizons® is a wellness program that offers you the opportunity to earn rewards for taking healthy actions.

Help accessing groceries and essentials

We all know the quality of food we eat affects our health. For many people, an unhealthy diet is a common reality.

According to Feeding America, more than 37 million people in the United States (or 1 in 9 Americans) are unable to access healthy food. Accessing healthy food, especially during the ongoing COVID-19 pandemic, has been difficult for many people. There are programs that can help.
