Louisiana Medicaid: Vision benefits

For Humana Healthy Horizons^® in Louisiana members under age 21, covered vision services include:

• Regular screening
• Eyeglass fitting, correction, and repair
• Eyeglasses and contact lenses
• Testing, care, management of eye disease
• Vision exams for eyeglasses and contact lenses
• Up to three pairs of glasses per year

Plus more from Humana, for members 21 and older:

• 1 annual eye exam
• Up to $100 allowance for 1 set of glasses (frames and lenses) or contacts during the plan year

Members 21 and older are responsible for any cost more than the $100 allowance.
