Welcome to Humana Healthy Horizons^® in Louisiana. Our members get hearing, medical, and vision coverage; behavioral health services; and extra benefits. You must get services from facilities and providers in our network. Providers in our network agree to work with us to make sure you get the care you need.

The only time you can get care from providers who are not in our network is for:

Emergency services
Federally qualified health centers/rural health clinics
Qualified family-planning providers
Any out-of-network provider we approve you seeing
