To find doctors accepting new patients, who speak your language and offer the services you need:

• Use our Find Care opens in new window service 
• Sign in using your Medicaid ID, date of birth, and ZIP code.
• If you are not yet a member, or do not wish to sign in, you can navigate to “Search as a guest” and enter your address, city, county, or ZIP code. 
  • Select the type of care you’re looking for.
  • Choose a medical network. Select “Humana Healthy Horizons^® in LA” 
  • Enter your search criteria

Find a dentist

Dental benefits for members 21 and older

Humana Healthy Horizons^® in Louisiana members who are 21 and older get dental services through DentaQuest.  

To find a dental provider:

• Use the online DentaQuest directory opens in new window (find a dentist)
• Select “Humana of  LA Adult Medicaid VAB”
• Click Confirm selection
• Call 1-800-685-0143 (TTY: 1-800-466-7566), Monday – Friday, 7 a.m. – 7 p.m.

Dental benefits for members younger than 21

Humana Healthy Horizons in Louisiana members who are under 21 can get dental services from either DentaQuest or MCNA Dental. You must choose one of these providers for your dental services.

To find a DentaQuest dental provider:

• Select “LA Medicaid EPSDT Dental Program” (under age 21)
• Click confirm selection

To find an MCNA dental provider:

• Enter your ZIP code
• Select “Louisiana EPSDT Dental Program”
