Provider directories

Our Find Care service includes real-time information about doctors in our network. Search for doctors, specialists, and practices in your area.

To find doctors accepting new patients, who speak your language and offer the services you need:

• Use our Find Care service opens in new window
• Sign in using your Medicaid ID, date of birth, and zip code. 
• If you are not yet a member, or do not wish to sign in, you can navigate to “Search as a guest” and enter your address, city, county, or Zip code.
  • Select the type of care you’re looking for. 
  • Choose a medical network. Select Humana Healthy Horizons in LA 
  • Enter your search criteria 

 

We update the below Provider Directories at the start of every month. You can view in English or in Spanish information about doctors in our network.

Dental benefits for members 21 and older

Humana Healthy Horizons^® in Louisiana members who are 21 and older get dental services through DentaQuest.

To find a dental provider:

• Use the online DentaQuest directory opens in new window (find a dentist)
• Select Humana of LA Adult Medicaid VAB
• Click Confirm selection
• Call 1-800-685-0143 (TTY: 1-800-466-7566), Monday – Friday, 7 a.m. to 7 p.m.

Dental benefits for members younger than 21

Humana Healthy Horizons in Louisiana members who are under 21 can get dental services from either DentaQuest or MCNA Dental. You must choose one of these providers for your dental services.

To find a DentaQuest dental provider:

• Select LA Medicaid EPSDT Dental Program (under age 21)
• Click confirm selection
• Call 1-800-685-0143 (TTY: 1-800-466-7566), Monday – Friday, 7 a.m. to 7 p.m.

To find an MCNA dental provider:

• Enter your ZIP code
• Select Louisiana EPSDT Dental Program
