Wellness programs for enrollees

We have programs for your unique needs. Wellness programs may help you improve your quality of life, have a healthy pregnancy or get behavioral health resources. Find programs to help on your path to well-being.

Quality enhancement programs

Many programs may help improve your quality of life. Your Humana medical plan can guide you to the programs available in your area. To learn more about these services, call your primary care physician or the Humana medical plan Enrollee Services team at 1-800-444-9137 (TTY: 711).

Domestic violence program

Your doctor should check for signs of domestic violence at your yearly visit. You can get educational information from your doctor.

Pregnancy prevention program

We can refer you to community pregnancy prevention programs that are open to you. You can use them no matter your age, gender, pregnancy status or parental consent.

Prenatal/postpartum program

We have a before and after pregnancy program. A case manager will help you through your pregnancy and give you learning materials. The program offers incentives, or ways to keep you on track, for healthy behaviors. When you have your baby, you will get more information about family planning from your case manager and your primary care doctor.

Children’s program

Humana Health Plan can refer you to children’s programs in your local area that help prevent health problems and give early healthcare services to families with children ages 0 to 5. Humana Health Plan also works with your primary care doctor to help keep your children healthy.

New medical treatment

Sometimes new treatments work well and sometimes they do not. Some can have negative side effects. We track new medical research, which helps us determine new benefits for your health plan. If you think a new medical technology or treatment might help you, call your primary care doctor. He or she will work with us to see if it can help you and will be covered.

Complex case management

Enrollees may be able to get complex case management services if they have been in the hospital a lot or have complex medical needs that require frequent, ongoing help. Complex case management is provided by specially trained Humana nurses and case managers. A team of doctors, social workers and community service partners helps make sure your needs are met. This team will try to help improve your overall well-being. The case management program is optional.

Behavioral health services

Behavioral health services are available to you without a referral from your primary care doctor.

You can find a behavioral health provider in your area. If this is an emergency, go to the nearest hospital or emergency room (ER).

myStrength

Your emotional health is important. myStrength is a safe and secure tool designed just for you. It offers personalized support to help improve your mood. myStrength’s proven tools can help strengthen your mind, body and spirit. myStrength is appropriate for enrollees 13 years of age and older. You can access this tool online or on your mobile device, at no cost to you.

myStrength offers:

  • Online learning topics such as depression, anxiety and substance abuse
  • Empowering self-help tools such as a mood tracker, thought and feeling log and a fitness log
  • Wellness resources such as mindfulness exercises, parenting tips, weight and stress management and smoking cessation
  • Inspirational daily quotes
  • A chance to establish goals and earn badges when goals are met
  • Articles and videos you can explore on a variety of different topics such as autism and attention deficit hyperactivity disorder

Ready to start?

  1. Visit myStrength, opens new window and click “Sign-Up.” Use access code: HUMANAKYMEDICAID if requested.
  2. Complete the myStrength sign-up process and personal profile.
  3. Go mobile! Download, opens new window the myStrength app for iOS and Android phones and SIGN IN using your login email and password.

Baby well-visit program

This program is for babies 18 months and younger. They must be members since birth. Call Humana at 1-800-611-1467 (TTY: 711) to learn more and sign up.

Pediatric well-visit program

This program is for children ages 2 to 21. Call Humana at 1-800-611-1467 (TTY: 711) to learn more and sign up. You can only sign up once per year.

Mom’s First prenatal/postpartum program

If you are pregnant, call Humana at 1-888-285-1121 (TTY: 711) to learn more and sign up.

Smoking cessation program

If you are 18 years or older, call Humana at 1-888-285-1121 (TTY: 711) to learn more and sign up.

Substance abuse program

Behavioral health services

Your behavioral health is as important as your physical health. If you need help finding a behavioral health professional in your area, please contact our case management team at 1-888-285-1121.

If the need for behavioral health care is urgent, you may call the 24/7 behavioral health crisis line at 1-833-801-7355 (TTY: 711) to talk to a behavioral health professional.

Behavioral healthcare professionals can talk to you about how to recognize the signs of a behavioral health condition and ways to manage your symptoms. Some of the conditions your behavioral health benefits may help with include:

  • Anxiety
  • Depression
  • Bipolar disorder
  • Schizophrenia/Schizoaffective disorder
  • Attention deficit hyperactivity disorder (ADHD)
  • Autism
  • Alcohol, drug and opioid use

You can get help with a behavioral health crisis 24 hours a day, 7 days a week by calling 1-833-801-7355 (TTY: 711). Your privacy is protected according to federal and state requirements.

Family planning services

With Humana Kentucky Medicaid, you can get family planning services from your current doctor. Or you can choose to get care from any doctor that offers family planning services. You do not need approval ahead of time. For more information, call the Humana Health Plan Enrollee Services team at 1-888-285-1121 (TTY: 711).

KidsHealth consumer web library

Find health and wellness information for the whole family! The consumer web library consists of over 6,000 pieces of education written for caregivers and families, specifically parents, kids and teens. You can find articles, videos, illustrations, wellness centers, recipes, games, quizzes and more.

The video library provides information about chronic and serious health conditions. In addition, personal stories help parents, kids and teens better understand their diagnosis.
