KidsHealth consumer library

The digital KidsHealth consumer library, opens new window has more than 6,000 pieces of education (e.g., articles, videos, illustrations, wellness centers, recipes, games, quizzes, and more) written for caregivers and families, specifically parents, kids, and teens.

The video KidsHealth consumer library, opens new window includes personal stories to help parents, kids, and teens understand medical diagnoses. The library has information about chronic and serious health conditions, including:

  • Asthma
  • Diabetes
  • Medications
  • Pain management
  • Seizures
  • Weight management

Wellness programs

We have programs for your unique needs. Refer to your Enrollee Handbook for information on the wellness programs we make available to our enrollees.

Quality enhancement programs and services

We offer our enrollees access to several quality enhancement programs. To learn more about these programs:

Our automated phone system may answer your call on Saturdays, Sundays, and some public holidays. This system will ask you to select the reason for your call, and we will call you back by the end of the next business day.

Our quality enhancement programs include:

Children’s program

We can refer you to children’s programs in your local area that help prevent health problems and give early healthcare services to families with children ages 0 to 5. We also work with your primary care provider to help keep your children healthy.

Chronic condition management program

We designed our chronic condition management program to help you understand and manage your chronic condition and follow your doctor’s plan of care.

Domestic violence program

Your doctor should check for signs of domestic violence at your yearly visit. You can get educational information from your doctor.

Moms First program

Taking care of yourself while you’re pregnant will help your baby have a healthy start. Humana Healthy Horizons™ in Kentucky helps pregnant and new moms get the care they need, makes available extra resources and services, and offers rewards for completing prenatal and postpartum checkups.

Pregnancy prevention program

We can refer you to community-based pregnancy prevention programs that you can use, no matter your age, gender, pregnancy status, or parental consent.

Baby well-being visit program

For enrollees 18 months and younger who have had Humana coverage since birth. Call Humana at 1-800-611-1467 (TTY: 711) to learn more and sign up.

Pediatric well-being visit program

For enrollees between the ages of 2 and 21. Call Humana at 1-800-611-1467 (TTY: 711) to learn more and sign up. You can sign up once per year.

Tobacco cessation program

Access to the Wellness Coaching Team using Go365 for Humana Healthy Horizons in Kentucky for participation in the tobacco- and vaping-cessation program.

  • For all enrollees ages 12 and older, up to 8 health coaching/cessation support calls within 12 months of the first coaching session
  • For enrollees ages 18 and older, nicotine replacement therapy upon request

Our quality enhancement services include:

Behavioral health services

When you or your family members need a place to turn for mental health and/or substance use issues, our behavioral health services can help. As your healthcare partner, we want to make sure you get the care you need to be healthy, so you can focus on the days ahead.

Complex care management services

Enrollees who have been in the hospital a lot or who have complex medical needs that require frequent, ongoing help may have the option to get complex care management services. Specially trained Humana nurses and care managers can:

  • Provide complex care management to you
  • Try to help improve your overall well-being
  • Work with doctors, social workers, and community service partners to help make sure your needs are met

Family-planning services

You can get family-planning services from your current doctor or from any doctor that offers family-planning services. You do not need approval ahead of time. For more information, call us at 1-888-285-1121 (TTY: 711).

New medical treatment

Sometimes new treatments:

  • Work well
  • Do not work well
  • Have negative side effects

We track new medical research, and use this information to help us decide to add benefits and services to your health plan. If you think a new medical technology or treatment might help you, call your primary care provider. He or she will work with us to see if it can help you and if we will cover it.

SafeLink mobile phone

Get a free Tracfone through the SafeLink federal program. Only one phone is available per household. Enrollees who are under 18 will need a parent or guardian to sign up.
