Vision services

Humana Healthy Horizons® in Kentucky enrollees have vision coverage.

All enrollees have coverage for:

  • Annual vision exam
  • Contact lenses if medically necessary
  • Eyeglass fitting
  • Testing, care, and management of eye disease

Enrollees younger than 21 also can get:

  • 1 pair of glasses per year
    • Additional pair covered if the first pair is lost, broken, or the prescription changes
    • Additional glasses may be provided through Early and Periodic Screening, Diagnostic and Treatment (EPSDT) special services

You may need approval before getting any of the above screenings and diagnostic testing services. This preapproval is called prior authorization.

Find an eye doctor

If you need an eye doctor, you can:

  • Use our Find a Doctor service to identify and locate eye doctors near you
  • Call Enrollee Services at 800-444-9137 (TTY: 711), Monday – Friday, 7 a.m. – 7 p.m., Eastern time.

Our automated phone system may answer your call on Saturdays, Sundays, and some public holidays. This system will ask you to select the reason for your call, and we will call you back by the end of the next business day.
