Chronic conditions

Humana Healthy Horizons offers resources as part of our chronic conditions program. A nurse can help you:

  • Find resources in your community
  • Get the education and support you need
  • Learn how to control diseases such as asthma, cancer, diabetes, and hypertension
  • Talk with your doctor

We designed our chronic conditions program to help you understand and manage your disease and follow your doctor’s plan of care.
