Everyone has unique needs and deserves unique care. Humana Healthy Horizons® in Kentucky is a Medicaid health plan built by Kentuckians for Kentuckians. You must renew your coverage each year. Renewal means you get to keep your coverage with us. If you don’t renew your coverage, you can lose your healthcare coverage and the extras you get from us.

Each year, you must renew your Medicaid eligibility. This is called “redetermination.” Redetermination is the process where the state checks to see if you still qualify for Medicaid. You must do this through your local DCBS county office. To find the office near you, go to www.chfs.ky.gov/agencies/dms .
You can complete the process in four easy steps. This is how it works:
1. Step 1: Prepare. Update your contact information. Make sure Kentucky has your current:
2. Step 2: Notice. You will get a letter from the Kentucky Department of Medicaid. This letter tells you that it’s time to check if you still qualify. The letter will include:

4. Step 4: Submit. Send the filled-out form back by the date in the letter. You should get the letter and form about 30 days before it is due. Return it right away so you don’t lose your benefits.

Humana Healthy Horizons in Kentucky cannot process your renewal for Medicaid coverage.
After you send in your information, the Kentucky Department of Medicaid looks at it. They check to see if you still qualify for Medicaid. They will make this decision based on the information you give them.
Then, you will get another letter. This one will tell you if you still qualify for Medicaid. If you do, your benefits will continue. If you do not, the letter will explain why.
If you don’t agree with the decision, you can appeal.
