Transportation services
All Humana Healthy Horizons® in Ohio members get 60 one-way trips (or 30 round trips) each calendar year. Rides are available for the following types of trips:
Non-Medical Transportation (NMT)
30 one-way trips (or 15 round trips) each calendar year for non-medical transportation needs including:
- Social support groups
- Wellness classes
- SNAP appointments
- Food banks
- Community and neighborhood centers
- Parks
- Baby showers
- GED® classes
- Grocery Stores
- County Department of Job and Family Services (CDJFS) appointments
- Maternity/childbirth classes
- Redetermination appointment
- Recreation areas
- Churches
Non-Emergency Medical Transportation (NEMT)
30 one-way trips (or 15 round trips) each calendar year for non-medical transportation needs including:
- Doctor, dental, and vision visits
- Members enrolled in Humana case management can qualify for additional transportation services, such as rides for:
- Intensive outpatient treatment
- Outpatient and residential behavioral health services
- Parents to visit their child in a residential or inpatient behavioral health facility
- Parents to visit their child in the neonatal intensive care unit
- Support new moms may need during the first 12 weeks after giving birth (trips cannot exceed 30 miles)
Unlimited transportation services
Unlimited transportation is available for members for the following services:
- Chemotherapy
- Diabetes management
- Dialysis
- Hospital discharge
- Organ transplant
- Postpartum trips (up to 12 months after delivery for doctor visits)
- Prenatal trips
- Radiation
- Urgent care
- Wound care
How to set up transportation
Transportation is provided by MTM Health. Please call MTM Health within 48 hours of needing a ride, if you think you need access to:
- A car or wheelchair
- Nonmedical transportation
Same-day transportation is available for urgent healthcare needs.
To set up transportation:
- Call MTM Health at 855-739-5986 (TTY: 866-288-3133), Monday – Friday, 7 a.m. – 8 p.m., Eastern time; or
- Use the MTM online portal/app

Note: Do not use the online portal/app for scheduling trips under 48-hour notice, please make sure to call MTM Health if you have an urgent healthcare need
To cancel a ride, call at least 24 hours in advance.
