Ohio Medicaid: Vision benefits

Humana Healthy Horizons^® in Ohio members have vision coverage with EyeMed!

General Medicaid benefits include:

• Comprehensive vision exam every year
• Eyeglass fitting, correction, and repair
• Eyeglasses and contact lenses
• Testing, care, and management of eye disease
• Vision exams for eyeglasses and contact lenses

Plus more from Humana, for members 21-59 years old:

• 1 annual eye exam
• Up to $200 allowance for 1 set of glasses (frames and lenses) or contacts during the plan year

Members are responsible for any cost more than the $200 allowance.

You may need to get approval (also called prior authorization or preauthorization) before using these benefits.
