Care management services

Our care management team can provide personalized support to our members who may benefit from care management. This support can include:

• Connecting members to resources in their community
• Coordinating healthcare services

We may reach out to you about care management if:

• Your doctor asks us to call you
• You ask us to call you
• We feel you and/or your family may benefit from this type of support

Once an member is enrolled in one or more of our care management programs, we can:

• Answer questions about any medicine you take
• Connect you with resources in your community
• Follow up with the doctors you see
• Help set up visits with your doctor(s)
• Help you figure out where to get the care you need when you need it
• Help you set and reach health goals
• Make sure you continue getting the care you need if you change doctors

We offer our members access to several care management programs. These programs include:

• Complex care management – For members who experience multiple hospitalizations or have complex medical needs that require frequent and ongoing help
• Disease management – For children and adults with chronic conditions who need access to integrative care services
• HumanaBeginnings™ care management program – For our pregnant members during and after their pregnancy

If you are hospitalized, our care managers can help you before you leave the hospital. We can:

• Answer questions you have about leaving the hospital or any medicine you’re prescribed
• Help set up doctor visits or any support you need at times that fit your schedule
