Children and adults with chronic conditions can access integrative care services that can help improve quality of life. These services can help members address potential co-morbidities and avoid complications.

Disease management programs

We make available programs to support our members who have one or more chronic conditions. These conditions include:

Asthma
Bipolar
Chronic obstructive pulmonary disorder (COPD)
Diabetes
Heart disease
HIV/AIDS
Hypertension
Schizophrenia

Integrative care services

For our members in one or more of our disease management programs, we can:

Give you tips on healthy lifestyle choices
Give you tips on how to take care of yourself
Help you understand how to keep your disease from negatively affecting you or your life
Help you understand your disease and how to control it and/or symptoms
