Humana Healthy Horizons® in Florida helps our pregnant and new moms get the care they need. Through our HumanaBeginnings™ care management program, eligible members get access to care managers, rewards for certain healthy behaviors, additional support, and more.

Health Risk Assessment (HRA)

We ask all members to complete an HRA. The HRA is your chance to:

  • Tell us about your health and well-being
  • Tell us about your health goals
  • Share information about your health-related needs
  • Give us an opportunity to pair you with a care manager who can help you reach your health and well-being goals

Completing your HRA is important for our pregnant members, especially during the first 60 days of enrollment in Humana Healthy Horizons in Florida. To complete your HRA:

  • Fill out the HRA and send it back to us in the envelope you got with your New Member Welcome Kit, or
  • Submit your HRA through Go365 for Humana Healthy Horizons® and earn:
    • $20 in rewards (if you complete the entire HRA within the first 90 days of enrollment), or
    • $10 in rewards (if you complete the entire HRA after the first 90 days of enrollment)
  • Call us at 855-351-7877 (TTY: 711), or
  • Register for a MyHumana account and submit the HRA online

This reward is available once per new enrollment in Humana Healthy Horizons in Florida.

HumanaBeginnings

We know all moms need help before and after giving birth. Our HumanaBeginnings program:

  • Is for our pregnant members
  • Includes rewards for going to the doctor and getting certain screenings and vaccines
  • Provides the help we know you need before and after you give birth

Members in our HumanaBeginnings program get:

While you’re pregnant:

  • Support from nurses trained to help during pregnancy
  • Referrals for other treatment and community services
  • Education and support for you and your partner or support systems
  • Transportation to and from doctor appointments
  • Extra help or support with understanding your Humana Healthy Horizons in Florida Benefits

After your baby is born:

  • Coordination with your hospital, lactation, and newborn team
  • Benefit information
  • Newborn support care when needed
  • Referrals for other treatment and community services
  • Family-planning information and services
  • Education and support from partners
  • Transportation to and from doctor appointments

Members in our HumanaBeginnings program can:

  • Earn rewards
  • Get a free hospital grade or non-hospital grade breast pump, if their OB-GYN submits a referral to a contracted vendor
  • Get their son circumcised, if so desired, at no cost
  • Access nutrition counseling sessions
  • Get meals delivered to their home after their baby is born
  • Get money back every month to use for over-the-counter medicine

Babies, children, and adults should see a dentist twice a year. Brushing, flossing, and avoiding sugary food and drinks can help keep your teeth and gums healthy. Problems with your teeth and gums can affect your overall health.

You know your body best

During and after pregnancy, you and your body will go through many changes. Some changes are not normal. They could be a sign of serious complications that can cause long-term effects or even death.

You may not know how to tell the difference between normal and abnormal changes—but you know when something is wrong. If you are concerned about symptoms or changes, talk to your doctor to make sure you get the care you need.

If you are pregnant or have been pregnant within the past year, and have any of these urgent maternal warning signs get medical care right away.

During a pregnancy, you and your OB-GYN will talk about your ideal birth plan. In other words, do you want to deliver vaginally or via a C-section? Here is some information that can help you make the decision that’s right for you.

What is a full-term pregnancy?

A full-term pregnancy is a pregnancy that lasts between 39 weeks and 40 weeks. Babies born full-term have the best chance of being healthy, compared with babies born earlier or later. Babies are not fully developed until 39 weeks of pregnancy. Their brain, lungs, and liver are still developing.

What are the benefits of vaginal delivery?

Vaginal delivery avoids:

  • Major abdominal surgery, and
  • All the potential risks associated with an operation

Following a vaginal delivery mothers:

  • Are more alert
  • Experience less pain and fatigue, and
  • Heal more quickly and spend fewer days in the hospital compared to mothers who have a C-section

Babies born vaginally typically:

  • Are more alert
  • Experience less respiratory problems
  • Have less chance of infection
  • Receive an early dose of good bacteria to boost their immune systems and protect their intestinal tracts, and
  • Show more interest in breastfeeding

When is delivery by C-section medically appropriate?

The more common medical reasons when a C-section is appropriate include:

  • Abnormal position of baby, or other type of distress or complications, such as with the placenta
  • Baby is too large for mother’s pelvis
  • Certain birth defects
  • Mother is experiencing very long labor of more than 20 hours with little dilation of the cervix, or is more than 41 weeks pregnant
  • Multiple births (mother pregnant with twins, triplets, etc.), and
  • Previous uterine surgery, including previous C-section

What are the risks associated with delivery by C-section?

Women who deliver by C-section tend to have:

  • A slower return to normal activity
  • Delayed or difficulty breastfeeding
  • Increased pain and fatigue
  • Longer hospital stays, and
  • Medical complications

And did you know that 90% of women who deliver via C-section during a first pregnancy deliver via C-section during subsequent pregnancies?
