When you first qualify for Medicaid, you may be assigned a health plan, but you don’t have to keep it. You have the right to choose a different health plan and switch to it during the first 90 days of your enrollment.

Medicaid eligibility in Louisiana

In Louisiana, you qualify to receive Medicaid if you:

You also may qualify for Medicaid coverage if you:

  • Are disabled according to the Social Security Administration’s definition
  • Have corrected vision no better than 20/200
  • Are a parent of children under age 19 and meet income requirements
  • Are a child under age 19
  • Are pregnant
  • Have no insurance and need treatment for breast and/or cervical cancer
  • Receive Medicare coverage and meet income requirements, or
  • Are aged 19 to 64 years old, meet income requirements, don't qualify for other Medicaid or Medicare coverage, and meet citizenship requirement

To help you get the most out of your Medicaid benefits:

Enrollment

LDH will notify you if you are a mandatory Medicaid member required to enroll in a plan. If you do not enroll in a plan, LDH will assign you to a plan. Once you are enrolled in a plan, you have 90 days to try out the plan. During this 90-day period, you can change your plan for any reason. After this 90-day period, you will remain enrolled in the plan, as long as you are eligible for Medicaid. This is called the “lock-in” period.

How to change your plan

If you are already enrolled in a plan and want to change to a different plan, you must meet 1 of the following requirements:

  • You have been a member of your current plan for 3 months or less, or
  • You’re having problems finding the care you need

If you do not meet 1 of the requirements above and still want to change your plan, you may change your plan during [open enrollment each year.

Disenrollment

If you are no longer eligible for Medicaid, you will be disenrolled from your health plan. You can lose Medicaid eligibility for a number of reasons, such as if your income level changes. We will notify you in writing if you lose your Medicaid eligibility and no longer have coverage through Humana Healthy Horizons in Louisiana.

You also can be dis-enrolled from Humana Healthy Horizons in Louisiana if you:

  • Abuse or harm health plan members, providers, or staff
  • Become eligible for Medicare
  • Do not fill out forms honestly or do not give true information (commit fraud)
  • Lose your Louisiana Medicaid eligibility
  • Stay in a nursing home for more than 30 days in a row

If you become ineligible for Louisiana Medicaid, all your services may stop. If you believe you are deemed ineligible in error, call the Medicaid Customer Service line at 1-888-342-6207 or the Enrollment Broker at 1-855-229-6848 (TTY: 1-855-526-3346), Monday – Friday, 8 a.m. – 5 p.m. to reapply.
