  • Submit answers to some health questions

We will use your answers to help make sure you get the care and support you need.

We also offer 24/7 access to your plan information with the MyHumana app. With it, you can:

  • See and download your Member ID card
  • Pick and change your primary care physician
  • Get important health notices
  • And more

Next, help us get to know you. Your Welcome Kit includes:

  • Some important health questions,
  • A Consent for Release of Protected Health Information, and
  • A Continuity of Care form

Please review and complete the forms, and then send them back to us in the enclosed postage-paid envelope.

We’ll use this information to help with your personalized care plan.

Answering the questions earns you rewards through Go365 for Humana Healthy Horizons.

Finally, it’s time to pick a doctor.

Your Member ID card includes the name of your primary care physician. To pick a different PCP, use our online Find a Doctor service at Humana.com/FindADoctor to see doctors in your area, change your PCP through your MyHumana account, or call the number on the back of your Member ID card.

Remember:

  • Pick and see your PCP

1. Download two digital apps to your mobile device

MyHumana

MyHumana, PDF is an online member portal account where you can find plan information and other tools to manage your plan. Registering:

  • Is easy
  • Takes only a few minutes

MyHumana lets you:

  • Download and print a copy of your member ID card in the ID card center
  • Set your communication preferences (e.g., you can get messages by email, text, or phone)
  • Chat with Member Services
  • Change your primary care physician

2. Fill out and return three forms

  • The Health Needs Assessment (HNA) is a list of health questions. Your answers can help us get to know you and make sure you get the care and support you need.

Please write your first and last name on each page before sending us your completed HNA.

You also can complete your HNA by:

  • Using the Go365 for Humana Healthy Horizons app, or
  • Activating your MyHumana account and completing and submitting the HNA online (available on desktop only), or
  • Calling us at 1-800-448-3810 (TTY: 711), Monday – Friday, 7 a.m. – 7 p.m.

All members can earn $30 in rewards through Go365 for Humana Healthy Horizons after completing the HNA during the first 90 days of enrollment in Humana Healthy Horizons in Louisiana.

  • Use the Continuity of Care Form to tell us about any medical conditions you have, so we can share that information with your doctors and make sure you get the care you need

  • Use the Consent for Release of Protected Health Information Form to give us permission to send your medical records to your doctor, if you change your doctor at any point during your enrollment

3. Pick and see a primary care physician

You will get most of your healthcare services from your primary care physician (PCP). You can pick your PCP. Use our Find a Doctor service or online provider directories to look for:

  • A primary care physician (PCP) near you
  • Specialists, hospitals, healthcare facilities, and other in-network providers

After you pick a PCP, let us know:

  • Submit the information through your MyHumana account
  • Call us at 1-800-448-3810 (TTY: 711), Monday – Friday, 7 a.m. – 7 p.m.

To use our online Find a Doctor tool go to Humana.com/FindaDoctor.

Enter your ZIP code, select Medicaid as your coverage type, and select Network from the drop-down menu.

You can then search by the name of the doctor or facility, specialty, such as cardiology, or a condition such as ear infection.

You also can choose All and type any text into the search.

Then click Search.

Click Update my search to search for something else.

If you need help finding and choosing a doctor, call the number on the back of your enrollee ID card to speak with an Enrollee Services representative.
