Your primary care physician (PCP) is the doctor or other healthcare provider that delivers the majority of your preventive care and services. You get to pick your PCP and can change it at any time.

Your PCP may send you to other providers or specialists, or admit you to a hospital, if needed.
You can reach your PCP by calling the PCP's office. Your PCP's name and telephone number are printed on your member ID card.
Use our Find a Doctor service to find a doctor who can serve as your PCP
To use our online Find a Doctor tool go to Humana.com/FindaDoctor.
Enter your ZIP code, select Medicaid as your coverage type, and select Network from the drop-down menu.
You can then search by the name of the doctor or facility, specialty, such as cardiology, or a condition such as ear infection.
You also can choose All and type any text into the search.
Then click Search.
Click Update my search to search for something else.
If you need help finding and choosing a doctor, call the number on the back of your enrollee ID card to speak with an Enrollee Services representative.
You can change your PCP when you need to. Please make sure we know the name of your PCP. If you try to get primary care services from a doctor we don’t have on file as your PCP, the doctor:
