Members with chronic or multiple health conditions can get help through one of our disease management programs.

Disease management programs

We make available programs to support our members who have:

Asthma
Bipolar disorder
Chronic obstructive pulmonary disorder (COPD)
Diabetes
Heart disease
HIV/AIDS
Hypertension
Schizophrenia

These programs may help a member:

Improve their quality of life
Address and avoid potential complications

Integrative care services

Members enrolled in a disease management program get:

Help learning how to keep your disease from negatively affecting you or your life
Help understanding your disease and how to control it and/or symptoms
Tips on making healthy lifestyle choices
Tips on taking care of yourself
