Keep your contact information updated

Ohio Medicaid needs updated contact information for you, including your address, phone number, and email address. Otherwise, you may miss important updates about your health insurance and lose your coverage. To update your contact information, sign into your secure Ohio Benefits self-service portal account, call a case worker through your local Department of Job and Family Services, or call the Ohio Medicaid Consumer hotline at 800-324-8680, Monday – Friday, from 7 a.m. – 8 p.m., and Saturday, from 8 a.m. – 5 p.m.

Your Quick Start Guide

All members get a Quick Start Guide in the mail. This Quick Start Guide includes the information you need to make the most of your enrollment in Humana Healthy Horizons in Ohio. All members also get a member ID card in the mail. Your ID card will arrive separate from your Quick Start Guide.

We can provide better care when we know more about you. You can help by filling out and sending back to us a Health Risk Assessment (HRA).

The HRA is a list of health questions. Your answers can help us get to know you and make sure you get the care and support you need.

Please write your first and last name on each page before sending us your completed HRA.

You also can complete your HRA by:

  • Using the Go365 for Humana Healthy Horizons app, or
  • Activating your MyHumana account and completing and submitting the HRA online (available on desktop only), or
  • Calling us at 877-856-5702 (TTY: 711), Monday – Friday, 7 a.m. – 8 p.m., Eastern time.

Earn $25 in rewards through Go365 for Humana Healthy Horizons if you complete your HRA during the first 90 days of enrollment in Humana Healthy Horizons in Ohio. This reward is available to all members.

MyHumana

MyHumana is an online member dashboard where you can find plan information and other tools to manage your plan. Registering:

  • Is easy
  • Takes only a few minutes

MyHumana, PDF lets you:

  • Download and print a copy of your member ID card in the ID card center
  • Select and change your primary care physician
  • Set your communication preferences (e.g., you can get messages by email, text, or phone)
  • Chat with Member Services

When creating your MyHumana account, you’ll be prompted to enter your Member ID. Your Member ID card includes your Member ID number. It is a number that begins with the letter H. Your Member ID card lists your Member ID as your Plan ID. Use this number when activating your online profile.

Go365 for Humana Healthy Horizons is a wellness program that offers you the opportunity to earn rewards for taking healthy actions. Humana Healthy Horizons in Ohio members can participate in Go365 for Humana Healthy Horizons. It is easy to participate in healthy activities and earn rewards through our Go365 for Humana Healthy Horizons wellness program.

Health and wellness

We want you to achieve your best physical, mental, and emotional health, so you can focus on the things you love. Humana Healthy Horizons in Ohio members have access to behavioral health services and health and wellness programs to help you on your journey to good health.

Use our Find a Doctor service to look for healthcare providers, specialists, hospitals, healthcare facilities, and other in-network providers.

  To use our online Find a Doctor tool go to Humana.com/FindaDoctor.

Enter your ZIP code, select Medicaid as your coverage type, and select Network from the drop-down menu.

You can then search by the name of the doctor or facility, specialty, such as cardiology, or a condition such as ear infection.

You also can choose All and type any text into the search.

Then click Search.

Click Update my search to search for something else.

If you need help finding and choosing a doctor, call the number on the back of your enrollee ID card to speak with an Enrollee Services representative.
