About Medication Therapy Management (MTM)

Medication Therapy Management (MTM) is a federally designed program sponsored by Humana that helps Medicare members to:

  • Know more – about getting the greatest benefit from their medications
  • Reduce risk – by learning how to avoid harmful side effects
  • Save money –by finding lower-cost alternatives to prescribed medications

All MTM members receive monthly mailings on topics like compliance, medication profiles and questions to ask their pharmacist. Some are eligible for a consultation—face-to-face or by phone—with a health professional.

Member eligibility

Humana use members' proprietary information to determine eligibility for the MTM program, like:

  • Medications: the number of medications they take
  • Medication costs: anticipated drug costs of $3,919 per year or more
  • Health condition: the number of diagnosed chronic illnesses they have
  • Plan enrollment: members must be enrolled in either a Part D Prescription Drug plan or a Medicare Advantage plan with prescription drug benefits

Membership is for an entire calendar year and is re-evaluated annually.

Join Humana's MTM Network, Managed by Outcomes

Humana has contracted with OutcomesMTM to deliver MTM services to eligible members in the community pharmacy setting. The Outcomes-MTM provider network includes chain and independent pharmacies, as well as hospital-based clinical and consultant pharmacists.
